Can no longer use controller in Fallout 4?
Had this issue and it drove me nuts! Check if your controller is properly connected or try a different USB port. Sometimes it's the simplest things. Also, dive into the game settings and make sure 'Controller Enabled' is actually turned on. Fallout 4 tends to reset preferences for no good reason.

Did you accidentally press something on the keyboard? Fallout 4 can switch to keyboard mode and disable the controller. Happened to me. Just unplugging and replugging the controller back in while the game was running fixed it.
Just restart your game and PC. Fixes 99% of problems with games not recognizing controllers.
Controllers are overrated. Keyboard and mouse give you better aim and faster reactions anyway.
Mine started working after I updated my controller's drivers and made sure the game was updated. Also, double-check your game settings!
