Absolutely, you can only adapt a creature in MTG if it has no +1/+1 counters on it. Once it's got counters from adapting or any other action, that particular adapt ability can't be used again on that creature. So, think of adapt as a one-shot boost per creature, unless you find a way to remove those counters after the fact!

Actually, you can't adapt the same creature twice if it's already got counters from adapting. But hey, there are ways to play around this if you're crafty! Removing the counters or adding them through different means is key. Adapt's like a "use it wisely" gym pass鈥攐nce per customer unless you've got a trick up your sleeve.
