Absolutely they do! In Magic The Gathering, whenever a planeswalker enters the battlefield, it comes in with a set number of loyalty counters as specified on its card. If that card leaves the battlefield for any reason (think exile, back to your hand, graveyard, etc.) and then comes back, it's considered a brand new object with no memory of its previous state. So, it resets to its original loyalty counter value. It’s like hitting the refresh button on your browser - everything starts anew!

Nope, they don't carry over. Once a planeswalker leaves the play area, if you bring it back, it's reset city for those loyalty counters.
