They didn't just pop up outta nowhere, lol. Feral ghouls are what happens when someone gets a mega dose of radiation and survives—kinda. No vault for them meant no protection. So, as you're trotting through West Virginia, remember: that ghoul was someone's neighbor once.

Actually, it's because Bethesda decided to include them as enemies for gameplay diversity. They're a staple of the series, after all. It's not super lore-friendly since the game's set only 25 years after the war, but hey, who doesn't love blasting ghouls?

Feral ghouls in Fallout 76, like in other Fallout games, are humans who were exposed to intense radiation without the protection of Vault-Tec's vaults. This radiation didn't kill them but turned them into the ghastly creatures you see. Over the 25 years since the bombs fell, these folks didn't have the luxury of a Vault 76 lifestyle and thus, went feral. It's all part of the charm of exploring the wasteland!
