Super mutants in Fallout 76 are there because of the FEV (Forced Evolutionary Virus) experiments that happened pre-war and continued by the Enclave post-war. In West Virginia, specifically, they were created due to experiments at the West Tek research facility, which got hit during the Great War, releasing FEV into the environment and infecting the local population.

Lore-wise, the super mutants in Fallout 76 come from experiments gone wrong. They were originally humans that got exposed to the Forced Evolutionary Virus. So, basically, science experiments turned them into what they are today.
