However, if a card creates two separate tokens or has an effect that splits it into two creatures under certain conditions, each can attack separately if they're not restricted by summoning sickness or other effects.

Short answer: you don't. A card that shows two creatures is still one creature in the game. Unless it specifically says it splits into two attackers, it's just one creature with a fancy picture.
Actually, there are cards like 'Bramble Sovereign' that can kinda make it feel like you're attacking with two creatures from one card. I used it to double my creatures as they enter the battlefield. Not exactly 'attacking with two creatures from one card' but it's the closest cool trick I've pulled off in a game.
