Head over to the Square Enix Account Management System, log in, then navigate to 'Account Information' and you'll find an option to unlink your PSN account. Easy peasy!

I had to unlink mine last month, and all I did was log into my Square Enix account, went to 'Account Information,' and unlinked it under 'Linked Accounts.' It took all of five minutes.
