Easy peasy! Just log into your main PSN account on the PlayStation website or console. Navigate to 'Account Management,' select 'Family Management,' then 'Add Family Member.' Follow the prompts to create a sub account for your kiddo or anyone else you'd like to add.

You can’t create a sub account anymore. Sony replaced them with Family accounts. Just create a child account under Family Management and you're good to go.
