You can't design items directly in the game, but you can participate in the TennoGen program on Steam to create custom skins and other cosmetics.
I tried my hand at designing a Warframe skin through TennoGen, and though it didn't make the cut, it was a fun and rewarding experience! Plus, the community feedback was awesome.
Just play the game and leave item design to the pros. Seriously, TennoGen is a tough crowd.
