You have to go full Genocide Route. The Real Knife appears in New Home after you've cleared the CORE. Trust me, it’s worth it!

To get the Real Knife in Undertale, you need to be on the Genocide Route. You'll find it in Asgore's house in New Home, replacing the Worn Dagger.
