In the end, Joel rescues Ellie from the Fireflies' hospital, preventing them from performing a fatal operation to create a cure. He lies to her, saying they stopped looking for a cure. Heartbreaking and thought-provoking!
Joel just straight up lies to her face about the Fireflies and the cure. Such a gut punch of an ending!
