You can swap out KO'd characters by going into the menu, selecting 'Formation,' and then moving your active characters around. Voila, back in action!

Just go to the menu, choose 'Formation,' and replace your knocked-out character with a reserve member. Easy peasy.
Hit up the menu, go to 'Formation,' and slide in a fresh character for your KO'd ally. Simple as that!
In battle, use the left trigger (L2) to switch out KO'd characters with reserves. Saved me a ton of times!
You really don't need to swap KO'd characters. Just revive them with items or spells. Keeps your team stronger overall.
