In my experience, isopropyl alcohol (at least 90%) on a microfiber cloth does the trick. Wipe from center to edge. Saved my 'Halo' disc once!
Grab a soft, lint-free cloth and gently wipe the disc from the center to the edge in straight lines. Avoid circular motions!
Honestly? Sometimes, it's just easier to install the game digitally if possible. Less hassle, and no scratches!
Mix a bit of mild soap with water, lightly dampen your cloth, and clean the disc as described. Make sure to dry it completely before popping it back in the console!
