Get down and dirty with as many people as possible. Eventually, you'll catch something nasty. No protection is your best friend here! Just don鈥檛 let your parents find out.
First, make sure your character is living in a polluted country. Then, keep committing crimes and get sent to prison. Once inside, don't seek medical treatment and you'll eventually die from a disease.
Honestly, just sleep around until you get sick. It's kinda grim, but hey, no one said being patient zero was glamorous.
