Easy peasy! Just turn on your Bluetooth on both devices, hold down the PS button and Share button on your controller until the light bar starts flashing, then select 'Wireless Controller' on your phone's Bluetooth menu. Bam! You're connected.

I use my PS4 controller all the time with my phone. Hold the PS and Share buttons at the same time until it starts blinking. Then, go to Bluetooth on your phone and pair it with 'Wireless Controller'.
