PS4 doesn't support most Bluetooth headsets directly. You may need a Bluetooth dongle.

Make sure your Bluetooth headset is in pairing mode, then on your PS4 navigate to Settings > Devices > Bluetooth Devices and select your headset from the list. Easy peasy!

Took me a while but: Settings > Devices > Bluetooth Devices. Then make sure your headset is in pairing mode!

Go to Settings > Devices > Bluetooth Devices, then put your headset into pairing mode. The PS4 should detect it, and you just select it from the list. Voila!
