You don't really create a team. Once you hit level 5, you pick one of the three existing teams at any Gym.
It's super easy! Reach level 5, go to any Gym, and then you can choose between Team Instinct, Team Mystic, or Team Valor.
When you hit level 5, you get to choose a team! Just visit a Gym, and Professor Willow will introduce you to Spark (Team Instinct), Blanche (Team Mystic), and Candela (Team Valor). Pick one, and you're good to go!
