Go to Settings > Initialization > Initialize PS4, then select 'Full' to wipe your system completely. Just make sure to back up any data you want to keep before hitting that button!
If you're looking to start fresh, head to Settings > Initialization > Initialize PS4. Choose the 'Full' option to delete everything. WARNING: This will remove all your data, so back it up first!
I had to do this a couple of months ago. Just go to Settings, then Initialization, then Initialize PS4, and choose 'Full'. It worked like a charm, but make sure you have your saves backed up!
