Hit Options on your controller, go to Equipment > Gear, and slot your scanner. During a mission, use the D-pad to bring up the gear wheel.

Easy peasy! Just pause the game, go to your gear wheel, and add the scanner there. Then, access it through the gear wheel during missions.
