Go to Settings > Initialization > Initialize PS4, then choose Full Initialization to wipe everything. Don't forget to back up your data first!

If you're selling or giving away your PS4, go to Settings > Initialization > Initialize PS4 > Full initialization to erase all your data. Backup your saves before!
