In my experience, echo happens from having game audio too loud in the same room. Try muting the TV or using headphones only.
You might have an echo because of a bad connection. Unplug and replug your headset, or try a different controller port.
Quick fix: Disable 'Mic Monitoring' in your Xbox settings. It usually gets rid of that pesky echo!
