When my PS4 got stuck in Safe Mode, I fixed it by connecting a USB drive with the latest update file from the PlayStation website. Just pick 'Update System Software' and follow the instructions. Worked like a charm!
Hot take: If none of the Safe Mode options work, you might just need to back up your data and try initializing your PS4. It's a bit extreme, but it resets the system to factory settings.
