Keep falling behind on purpose! The Bullet Bill item tends to appear in the item boxes only when you're near the rear of the pack – around 7th or 8th place. Just don't fall too far behind, or it'll be game over!

Stay in those last positions! Bullet Bill is more likely to appear when you're struggling near the end. I always get it when I'm suffering in 7th or 8th place.
