Easy peasy! To get Leafeon, use a Mossy Lure Module at a PokeStop and evolve your Eevee. For Glaceon, use a Glacial Lure Module at a PokeStop and evolve your Eevee. Make sure you're close to the PokeStop when evolving!

Just nickname your Eevee 'Linnea' for Leafeon and 'Rea' for Glaceon before evolving! Remember, this trick only works once for each evolution, so use it wisely!
