You can get a Name Change Card by purchasing it from the in-game shop using G-Coins. Just head to the shop, look for the 'Others' section, and you'll find the card there!

In my experience, the easiest way to get a Name Change Card is straight from the shop. It's around 180 G-Coins, and boom, new name!
