You need to complete 'The Second Dream', 'The War Within', and 'Chains of Harrow' quests. Then, the quest should be available in your Codex.

To start 'The Sacrifice' quest, you need to have completed 'The Second Dream', 'The War Within', and 'Chains of Harrow' quests. Once you're DONE with those, visit your Codex and it should pop up!
