Why link them? If you know someone is your patron, just ask them to tell you their Discord username!
Just did this last week! Go into Patreon, find the 'Account Settings', and click on 'Connected Apps'. Then hit 'Connect' next to Discord, and follow the prompts. Not rocket science, promise!
All it takes is a trip to your Patreon settings, hitting 'Apps', and picking 'Discord' to connect them. Simple as that!
