All you gotta do is basically do nothing. No gym, no swimming, no cycling. Also, let CJ become best friends with fast food. Muscle loss is guaranteed.

Actually, losing muscle in GTA is a waste of time. Keep that strength up; you'll need it for missions!

Just starve CJ for a few days, avoid the gym and any kind of workout. Muscle will go down slowly but surely. Seems counterintuitive in the real world, but hey, it works in San Andreas!

To lose muscle in GTA San Andreas, have CJ stop hitting the gym and avoid doing any physical exercise like swimming or cycling. Also, make him eat lots of junk food. It'll take some time, but you'll start to see those muscles shrink.
