First off, make sure your system meets the game's minimum requirements, cause even in a post-apocalyptic world, you can't outrun bad specs. Update your graphics drivers too; those drivers can be older than a Super Mutant and just as cranky. Also, try running the game in Windowed Mode instead of Fullscreen, and remember to close other programs that might be hogging your system's resources.
Mods are great but they can also be the cause of crashes. Make sure they're up to date and compatible with each other. Sometimes, the solution is as simple as disabling a few that conflict.
Honestly, just play on console. Saves you all the headache.
Did you try turning it off and on again? But seriously, sometimes a fresh reinstall does wonders. Just make sure to backup your saves first!
