Just grab 6 lenses from those pesky Demon Eyes that float around at night and mash 'em together at a Demon/Crimson Altar. Easy peasy.

Honestly, just buy it from the Merchant during a Blood Moon if you're not in the mood to hunt for lenses. It's way easier.

Making a Suspicious Looking Eye in Terraria is pretty straightforward but it'll have you delving into some dangerous places. You need 6 Lenses, which you can get from defeating Demon or Wandering Eyes at night. Once you've got those, head over to a Demon or Crimson Altar. These altars are usually found in the Corruption or Crimson biomes – spooky, dark places with a distinct vibe. Use the lenses at the altar, and boom, you've crafted a Suspicious Looking Eye! This item summons the Eye of Cthulhu, so make sure you're ready for a fight.
