Honestly, just fill out the merge request form from the Page settings. If your Pages are too different, though, Facebook might reject it.
From personal experience, you need to be an admin for both Pages鈥攖hen just head to facebook.com/pages/merge and follow the instructions. Make sure both Pages have similar names to up your approval chances!
All you need to do is make sure both Pages are yours, go to one Page's settings, find 'Merge Pages' option, then hit merge and you're golden!
