I've done this a dozen times: disconnect the controller, turn it off, and reconnect using the USB cable for a fresh start.

Just hold down the sync button (near the USB port) and the Xbox button at the same time until it turns off and restarts. Easy peasy!

Turn off the controller, then remove the batteries or disconnect the USB for about 10 seconds before powering it back on.
