To save Ryan, make sure to amputate his bitten leg right after he gets bitten by Chris. You'll need the chainsaw or knife. Also, make sure to form a good relationship with Laura鈥攕he'll need to help him survive.
It all comes down to amputation and keeping him stress-free. Couple that with making the right choices, and you're golden.
I saved Ryan by making him trust Laura early on and using the chainsaw on his leg. Pretty intense, but it worked!
