I had to go through Ubisoft Support to do this. Contact their customer service, and they'll help you out.

Just log into your Ubisoft account, navigate to 'Linked Accounts,' and click 'Unlink' next to PlayStation Network.

Go to Ubisoft's Account Management site, then log in. Under 'Linked Accounts,' find PlayStation Network and hit 'Unlink'鈥攕imple as that!
