First, die as Isaac to a Mulliboom in the Basement/Cellar. Then you need to die as Magdalene to your own bomb in Caves/Catacombs. Die as Judas in the Mom fight and finally, die as Azazel via a Sacrifice Room spike while holding the Missing Poster. Yup, it's a journey!
