Simply hold 'Alt'. It's like asking, 'How do I throw a party?' Easy, invite people and add music. For grenades, invite them with 'Alt' and add an explosion.
Equip the grenade, then hold 'Alt' to aim and release it to throw. Got my first super mutant kill this way!
