First, add the scanner to your gear wheel. Then, when in a mission, select it from the wheel, aim at what you want to scan, and hold the fire button until the scan completes.

Equip your Codex or Synthesis scanner in the gear wheel, aim at the target, and then hold the fire button to scan. Easy peasy!
