Killing even one monster affects your relationships with other characters and can lead you down the Neutral or Genocide routes.
If you kill one monster in Undertale, you'll get EXP and your LOVE (Level of Violence) will increase, which will affect the rest of your gameplay and endings.
First-hand experience: I played through Undertale and where I accidentally killed someone early on. It made Toriel super upset and messed up my chance at the true Pacifist ending.
