Cayo Perico is not actually a part of the main GTA 5 game - it's an island featured in the GTA Online: The Cayo Perico Heist update. It's the first ever map expansion for GTA Online. The island is a private paradise owned by the world's most notorious drug dealer. Players can infiltrate it solo or with a crew, in what's touted as the biggest heist yet in GTA, with new vehicles, tactical weapons, and a submarine HQ.

It's just a DLC, not in the base GTA 5 game. Worth it if you're into GTA Online.
