The rarest Japanese Pokemon card is widely considered to be the Illustrator Pikachu. It was given out to winners of a CoroCoro Comic illustration contest in the late '90s, and only a handful exist. Its uniqueness and limited availability make it a holy grail among collectors.

Don't forget about the Master's Key. It was awarded during the 2010 Pokemon World Championships in Japan. Super rare and definitely up there with the Illustrator Pikachu in terms of scarcity and value.
