First, try connecting it with a USB cable to see if it pairs. If that works, then go to the Bluetooth settings and retry pairing wirelessly.

Hold down the reset button on the back of the controller with a paperclip for a few seconds. Then, try reconnecting it!

I removed and re-paired my controller through the PS4's Bluetooth settings. Worked like a charm! Just go to Settings > Devices > Bluetooth Devices.
