Hold down the reset button on the back of the controller with a paperclip for a few seconds. Then, try reconnecting it!
Bro, just restart your PS4. Sometimes the old 'turn it off and on again' trick works miracles.
I removed and re-paired my controller through the PS4's Bluetooth settings. Worked like a charm! Just go to Settings > Devices > Bluetooth Devices.
