The main difference is that Fullmetal Alchemist (2003) diverges from the manga halfway through, while Fullmetal Alchemist: Brotherhood (2009) follows the manga storyline faithfully from start to finish.

I watched both. The original series has more filler content and diverges from the manga, but Brotherhood is faster-paced and sticks to the true story!
