In The Sims FreePlay, your Sim will get a baby bump after you complete the 'Nine Months Later' quest and your Sim is pregnant. It takes 6 days for the baby to be born, so the bump will appear gradually during this period.

To get a baby bump in Sims FreePlay, just finish the 'Nine Months Later' quest. Your Sim gets the bump right after that.
