Level 5 is in the top row of the map, dead center. Look for a tree you can burn down with the candle!
The fifth dungeon entrance is in the dead center of the map. Burn the middle bush in that cluster of bushes. Simple!
You need to use the Whistle by the pond in the top-center part of the map. Just make sure you have it in your inventory first!
