Each map in State of Decay 2 has 10 Plague Hearts. If you're having trouble spotting them, just scout from high vantage points until you find the red mist!
To find Plague Hearts, check for clusters of zombie activity on your map, and listen for the growling heartbeat sounds when you get close!
