Probably because your character's smarts are too low. Hit those books, or use that 'Meditate' option to boost 'em!

Firsthand experience: My character got into med school after acing high school, going to university for Biology, and having high smarts. Avoid bad behaviors and keep studying!

Check if you went to university for the right major, like Biology or Chemistry. Not all majors will get you into med school!
