Make sure your controller's firmware is up to date! An outdated firmware can cause connection issues. Check Microsoft's support page for a guide on updating.

If your controller keeps disconnecting only when playing certain games, it might be a software issue. Try reinstalling the game or checking for game updates.

Are the batteries low? Weak batteries can cause the controller to lose connection. Swap them out for fresh ones or use a rechargeable pack.

I had the same issue, and it turned out my USB cable was faulty. If you're wired, try a new cable and see if that fixes it.
