If you're trying to redeem a region-locked code, make sure your account's region matches the code's region.
I had this issue once. Turned out I had to just restart my console and try again. Worked like magic!
Call Microsoft support. They usually fix it pretty fast. It's annoying but effective.
Check the code's expiration date. It might be expired. Happens a lot more often than you'd think.
