It's rated R mainly for the extreme violence, intense combat sequences, and some strong language. They wanted to stay true to the game's brutal nature.
Why? Because you can't have Mortal Kombat without over-the-top fatalities and heads flying off. Duh.
I watched it, and trust me, the R rating makes sense. The fight scenes are just like the game – brutal and epic!
