It's not official yet. Season 1 was by Madhouse, Season 2 by J.C. Staff. We'll have to wait and see who takes up Season 3!

Season 3 animation studio isn't confirmed. Keep an eye on official sources for updates!

Most likely not. Different studios have handled different seasons, and there's no confirmation Madhouse will return.
